Hon Richard Yates Governor of Ills
Dear sir
I wrote you some time since asking for an appointment as Surgeon or assit. Surgeon in the Army and your reply was that I would first have to pass an Examination before a Medical Board. Now I would ask where that Medical Board is located, so that I may pass Examination. I would ask if after I pass Examination and report favorable would that be sufficient garrantee for a commission. I would also wish to know if a Surgeon furnishes his own Instruments & Medicine or dont The Government furnish them. Please give me the above information and you
