July 26th 1864

To his Excellency Gov. Yates,

Sir I see President - Lincoln has made a call for five Hundred thousand (500,000) volunteers to be raised by the 10th of Sept. next. I wish to serve my country where I can do the most good and where I am most needed. I have been in the U.S. Service as act. Asst. Surgeon USA, since the 6th day of August 1862. I would now join the Ridgment as Surgeon if you can give me a place. For the last three years preceding this Rebellion I resided in Antioch Lake Co, Illinois. I have two sons in the Army the eldest has served almost three years, in the 7 Battery Wisconsin Vol. The youngest aged 16 is one of the one Hundred days men a member of Co. F 134 Ridg. Ill. Vol. I would accept of a commission in either a White or Black Ridgment tho the former would be prefered.
