His Excellency Richard Yates Governor of Ill.

Sir: I have the honor to inform you that I am severely afflicted with dyspepsia, have been transferred various times from one hospital to another. My disease is gradually growing worse, and I have less than seven weeks left to serve, my discharge has several times been ordered, and I have failed to get in on account of the transfers - referred to above. Will you have an order issued for my transfer to Ill. as soon as possible when I can appear before any Board and get my discharge by the time my term has service has expired.

Without your aid I shall fail to get home at the expiration of my term.

I am sir very respectfully your obt. sevt. Charles G. Schultz Co. "G" 60th Ill Inf.
