Serving in an Illinois Regt in the battles before Atlanta I had the misfortune to get wounded Aug 11th 1864
The ball passed through my face going in behind my right ear and coming out below my left eye the Dr has told me that I will be unfit for duty for two months
Now I respectfully apply to you for a transfer to the U.S. Hospital at Chicago
