The undersigned willing and desirous to enter the volonteer-service of the Unidet States as a surgeon would respectfully solicet your Excellency to place his name on the list of the applicants for examination before the board of medical examiners. If testimonies of ability are required they can be furnished by the physicians of Menard County Ill, where the undersigned practiced. The undersigned also respectfully requests Your Excellency to send the order for examination to his adress care of Dr. Fr. Brendel Peoria Ill. Your Excellency's obedient servant
