His Excellency Gov Richd Yates Springfield Ills

Dear Sir

Holding a Surgeon's Certificate from the Board of Medical Examiners, I am induced to tender you my services, subject to any future contingency that may arise in connection with the wants of the Medical Department of the Army. Should you see cause to follow the practice of some of our loyal Governors who have supplied such regiment with an additional Surgeon, I shall be glad to receive a summons from you at any time.
