His excellency the Governor of Ills Richard Yates

Sir the call for nine Thousand more Solders from Ills will necessary create a demand for more Surgeons and Physicians. Believing it to be the duty of every loyal citizen to aid in puting down this Rebllion. I offer my servises as a Surgeon to the Army, ready and full equpt to march at any time and to any place As I have been in the Hospital in in St. Louis for some time I have some experience in the line of duty.
