Dear Sir

I take the liberty of addressing you in regard to myself I have Been now in service since the 8th of July 1861 and was in the 3 months service and have Been Principal Musician of the 28th Ills and I have Been faithfull to my Duties and have Been Deaf now in one of my Ear's for 3 months hearing nothing at all. And I wish to Be discharged from service or have a leave of absence for 15 Day's I refer you to the following officers as to my truthfulness.
