Having been discharged from the Survice on a count of a wound received in my left hand at the battle of Fort Donelson Disinabling me for the deutes of a private but wishing to a gain enter in the Service of our Country. I make this application for a commision in a Negro Company. I was Sworn in the U.S. on the 30th of July 61 in Geo C.
