To Hon Richard Yates -
John W. Van Schoick a private in Company A 94th Reg. Ill. Vol - is now very sick in Washington Hospital. Memphis- has been sick a long time. and his Brother from our city sends the Bearer George Hampton to try to get him home till he can recover his health. We ask of you the favor to recommend to the Post surgeon at Memphis that he grant him a sick furlough and we will be responsible for his return to his Regiment soon as he is able. he is a good faithful soldier and thinks he will die unless he can come
north long enough to gain his health again. We hope you will aid the Bearer in getting through to Memphis and give the proper recommend to the Post Surgeon. the Soldier is very anxious to get well so can kill a few more Rebels --
Yours Respectfully
