James Clements a private of Capt J J Heffernans Co (H) 55th Reg Ill Vol Having applied for a Certifficate on which to ground an Application for leave of absence. I do hereby certify that I have carefully examined said private and find that he is incapable of performing the duties of a Soldier because of General Debility and that in Consequence thereof he is in my opinion unfit for duty I further declare my belief that he will not be abled to resume his duties in a less period than Sixty days

TO WHOM IT MAY CONCERN:

The bearer hereof James Clements a Private of Captain JJ Heffernan's Company (H) 55th Regiment of Illinois, Volunteers aged 44 years five feet 9 3/4 inches high light complexion Blue eyes. Light hair and by profession a farmer; born in the state of New York, and enlisted at Polo in the State of Illinois, in the twenty ninth day of October eighteen hundred and sixty one to serve for the period of Three Years, is hereby permitted to go to Polo in the county of Ogle State of Illinois, he having received a, Furlough from the 22nd day of March to the 22nd day of May 1862 at which period he will rejoin his Company or Regiment at Paducah KY. or wherever it then may be or be considered a deserter.

Subsistence has been issued to said James Clements to the 22nd day of March 1862, and pay to the thirty first day of December 186one both inclusive
