The Undersigned have apointed themselves together as "The Florence Nightengale Corps" of nurses for the Army, with the humane & patriotic desire of being useful to their Country in its hour of trial, by ministering to the sick or wounded soldiers, and thus aiding in restoring them to health & duty, and in such Capacity they hereby tender their services at a days notice and whereever there may be occasion for them.
The undersigned have provided themselves with the proper & Requisite certificates from Clergymen & Physicians as to their moral standing, suitable age, health & qualifications as nurses and have chosen Mrs Phebe E. Yates of Chicago as their presiding Matron. And in the belief
that thier services must be needed whereever large bodies of men are congregated under changed circumstances, and that thier efforts will contribute to the relief of suffering and saving of life. They Respectfully request that an Early requisition may be made for thier services under such regulations as shall be thought proper.
