Sir:
I have the honor to request the appointment of myself as "Assistant Surgeon" in some Illinois Regiment; as I have a desire to go to the field.
I seek this position from the state of Illinois because my home is there; & because I once served as a volunteer from that State.
If there be such a vacancy and accompanying Recommendations are satisfactory, please address as soon as convenient,

This is to certify that Dr. A. G. Leslie, has been with me in these Brks under contract as A. A. Surg U. S. A. since the 1st of July last. and I take great pleasure in cordially recommending him to the favorable consideration of the Govnr of Ills. (the state from which he hails.) Dr. Leslie is a young gentleman of fair abilities, is very attentive to his professional duties, and a high-toned honorable gentleman.
I am of opinion that he is well qualified for the position for which he asks, and will acquit himself with credit to himself, benefit to the service, and honor to his state.
