I, John S. Winter, Clerk of the County Court in and for said County and State, do hereby certify that Wm. H. Camp Esquire, whose name is subscribed to the foregoing Jurat to Affidavit was, on the day of the date thereof, to-wit: on the 6th day of December 1864, an acting Justice of the Peace in said County and State, duly elected, commissioned and sworn, and, as such, duly authorized and empowered to administer oaths for general purposes, to take the acknowledgement of Deeds, and all other Instruments, in writing, and that his signature thereto is genuine.

In Testimony Whereof, I have hereunto subscribed my name, and affixed the seal of said County Court, at the City of Knoxville, this 14th day of December A.D., 1864

This is to Certify that I A.P. Boal a practicing Physician in said town Have Examined George H. Wagner, corp Co G. 89 Regiment Capt Whiteine Ills vol and find that from a Gunshot wound Receive accidentely While in the Service of the Government that said accident did fracture the ulna and from the attending inflamation or other cause the arm is perminantly disabled He being entirely unable to close the left hand that being the Hand of the [ingerd?] arme the Wound was received on the 11 of Feb, 1864 and believeing that he would be entirely unable to serve as a soldier [now?] ask in hope of receiving a discharge from the service for said Wagner.
