(?) Middleton Ills
Decr 28 1864
To his Excellency Gov Richard Yates. you will please pardon me for attempting to advise in a matter that is doubtless well understood by others. The Case is this Jacob W Fyke a Sergt I believe in Co E 111 Ills vols has been for years afflicted with Neuralgia in the Hip but owing to his patriotism enlisted. hoping to be able to do something for his Country But as might have been expected has Scarcely been able to do a days Service Since his enlistment, and is at this time in hospital in this state
It was my province to be Hospital Steward of his Regt. and am fully acquainted with his situation and am Convinced that he Cannot be of any Service to the army during the remainder of his term of enlistment and his discharge which would be of inestible value to his family would releeve him of the confinement of a Military Hospital Life
His family consist of a wife and three Helpless Children. If you can do any thing in his Case you will not only Confer a great favour on him but on his Numerous friends
Said J W Fyke is a gentleman in every Sense of the word. and would as willingly Serv out his time as any Man in the army if he Could be of any Service
please don't think that I am medling I am a Physician and well acquainted with the Case
Yours with due Respect
