Springfield June 30th 1864 Surgeon in Charge U.T. Hospitals Chattanooga Tenn.
Sir:
I am informed that John C. Purtle Priv Co H 125th Regt Ill Vol's, has been confined in Hospital No 8 Chattanooga for nearly four months and that there is but little hope of his even being fit for field duty. Please have him Examined as soon as possible with a view to his discharge, or if not a fit subject for discharge assist him in getting a furlough, to unable him to recover his health under the care of his friends.
Please return this letter with endorsement of such action as may be taken in the (premises?).
