Having recieved the appointment of Surgeon of this regiment from its officers and the commander of this department also leave of absence for eight days for the purpose of going before the Ill. State board of medical examiners I desire your permission to appear before said board for examination for promotion also timely notice of when and when and where I can find them in session. Some time between the 25 and 30 days of this month would favor me
