Dear Sir
About one year ago I had the honor to be commissioned by you as surgeon of the 2nd Illinois Cavalry, and since that date I have been constantly at my post, discharging its duties without the loss of a single day. From motives of a purely professional nature I desire to exchange my position in the Cavalry, to an infantry Regiment and make this application in the hope that you may be able to further my wishes, by appointing me to some one of the new Infantry Regiments about being organized for the war. My lengthened military experience would be of some value to a new regiment, and the Ass Surgeon Dr A.J. Crain having passed examination before the medical board of the state, is fully competent to be appointed Surgeon in my place.
As a considerable number of new surgeons will be required under recent call, I trust you may be able to take this application under consideration; and if proper in your view, commission me to one of the new Regiments.
I refer you to the enclosed letter, and also to those of the late major Applington and others, left on file in the State Dept one year since and remain with very great respect
