R.W. Bailey on behalf of the New York City Clerk, completes this partially-printed document and City Inspector F.J.A. Boole signs the authorization allowing Peter Relyea, the official undertaker for the City of New York, permission to remove the body of Abraham Lincoln, who died of a "pistol shot wound," from "the City" and transport it to "Springfield, Illinois for interment."
