Dr Sir Enclosed I send a tender of my services to governor Yates, as Surgeon or Assistant Surgeon to any Illinois Volunteers that are or may be called from this State. Of my ability to act in that capacity and to honorably discharge its duties, I think you are knowing to. Any assistance you can give me, in bringing my tender properly forward and giving me such recommendation as you think justifiable will be thankfully received by

To the Hon Richard Yates
Sir
I hereby tender my services as Surgeon or Assistant Surgeon to any volunteers that may be required from this State.
If the posts to the regiments now called are full I will hold myself in readiness to answer a call whenever made.
For my ability to fill the post I refer to the enclosed certificate and my freind Mr. Edward Kitchell of Olney, now in Springfield
