To His Excellency Richard Yates Governor and Commander in Chief of the Militia of the State of Illinois
Dear Sir Allow me to introduce to your notice [as?] competent and every way worthy [illegible] appointment as Surgeon Should his Services be required dr. L. P. Tibbetts of DuPage County. The Doctor is a thorough Scholar and has had much practical and professional acquaintance with Anatomy Physiology and Surgery. He is a man of middle age good habits, energetic character, has [been?] accustomed to frontier and Camp life. I can recommend him as one who if employed will be very servicable to our brethren in arms, whether in camp to direct their habits or following close the Field of Action to alleviate their Sufferings.
