September 26th 3 To Commanding Officer of U.S. Forces at Louisville KY. and of Forces in Departments of the Cumberland and Ohio

This will assure you that the bearer hereof is a worthy and loyal citizen of Illinois, Mr. Fielding L. Scott, who has given liberally in aid of sick & wounded soldiers and has two sons in the army of the Cumberland - one of them supposed to be mortally wounded - I urgently desire & request that he may be permitted to visit him & for this purpose that pass [for?] & within the lines of the army may be furnished him.
