U. S. Hospital No 1 Quincy Ills. Sept. 28th 1864
Hon,. R. Yates Governour of Illinois Sir I with to lay my case before you and hope you will ?? me if you think I am intitled (sic) to such after you have heard Mr. Yates. I entered the service in August 1862, I had my health and done my duty for about one year. While at Vicksburg I meat(sic) with an axcident (sic). I fell from the staging of a boat while purchasing Regimental Rations. Since then I have not been able to do my duty and several Surgeons have told me they can not cure me. I have been awarded my discharge By three different boards but have never been able to get my subscription Role therefore they could not give me a discharge. Governour, I have a Wife and four little children at home. I am a poor man and my family is suffering. My monthly wages , even if I could draw it, which I can not without a description Role is insufficient to support them at the increased prices for Produce. [illegible] if it has not been for the kindness of some of the neighbors they would have suffered for the common necessities of life long ere this. I makes my heart bleed to see those that I love better than life suffering and I lay here of no use to my country or family and all through the neglect of a few petty Officers. He says he has been in service two years. He got wounded at Vicksburg over a year ago. Has been in the hospital ever since. He could get a discharge if he had his Description Roll, wants it sent to him.
