His Excellency Richard Yates - Gov of the State of Illinois

Dear Sir

Being a descendant of the "Puritan Sires" - my granfather Maj. John Mills being in the battle of Bunker Hill of Revolution memory, I am anxious to do something to perpetuate the Goverment for which they fought and wish to serve in that Capacity - that I can do the most good - viz as Assistant Surgeon - And I would especially wish to be connected with the Reg to rendez vous at Kankakee two companies forming here


to go there - I would also say that I gave two weeks of gratuitous service to the 25th Reg Ill vol at St Louis Mo last year -

You have my certificate from the Medical board. And I would like to be commissioned so that I could say that I was going and have additional influence in urging recruits - I refer to [Lawrence?] Welden - James B. McKinly - Judge David Davis &c
