I have the honor to respectfully request that I may at your earliest convenience be furnished with the date of my Enlistment and also the date of my muster into service, that I may be mustered out.

I formerly belonged to Co "D" 7th Ills. Infantry, I Enlisted on or about the 15th of July 1861 and was mustered into the U.S. service July 25th 1861.
