His Excellency Gov. Yates

Dear Sir

please find enclosed my affidavit Setting forth a Statement of facts which I hope will be Sufficient to procure the discharge of my son, who on account of his feebleness of health has never been able to do his country any Service Since last Spring I believe if I had him with me he would be of great benefit to me, in my helpless condition. And I could also in my humble way add some little to the restoration of his health. You will confer a lasting favor on a helpless mother by forwarding the accompanying affidavit to the proper authorities with your recomendation that my son be Discharged from Service for the reasons therein stated.
