Fulton County } Lida Warner comes and on her oath Sais that her Husband Samuel. O. Warner is now in the Survis of the united States a Soldier in Company E of the 53rd Regment of Illinois voneteers under capton Bull Farnell name not known She further States that She is So afflicted that She is not able to Suport her Self and that She is Destitute of any means to Suport on and that She has one child three years old She further Stats that her Said husband age is forty years old and her age is twenty three years She makes this affidavit For the purpose of (allowing?) the discharge of her Said husband

Warner who is a private in Company. E. in the 53 Reagment of Illinois volenteers from any Further Service from the fact that his wife Lidia Warner is affected So that She is not able to Suport her Self
I hereby verify that the above mentioned Lidia Warner is afflicted with a serious chronic malidy which is incurable and which renders her inable to support herself
