J. O. Harris M.D. being duly sworn saith on oath that on or about the 14th day of March AD 1863 affiant was called upon by William S. McCormick to examine his son Bruce McCormick who was then home on a furlough. I found him unfit to do military duty and gave him a certificate of those facts which certificate was in the usual and proper form to enable him to procure an extension of his furlough. he was then suffering from paralysis and chronic diarhaea, according to affiants recollection now. Affiant is a physician and surgeon in regular practice and was from 1861. to Nov 20 1862 assistant surgeon of the 53rd Ill vols

I Samuel W. [Raymond?] Clerk of the County Court of said county do hereby certify that Doctor J. O. Harris whose name appears signed to the above certificate appeared before me
at my office and made oath that the things contained in said certificate are true, and further that said Harris is a regular physician residing in the City of Ottawa in said county.
