Sir: I would respectfully request you to examine Mr. Jacob Heyman Co "C." 82nd Reg't Ills Vols. for discharge.
He has been in hospital since June last and his left hand is entirely useless to him.
Please examine him as soon as possible and render him all proper assistance to procure a discharge.

Respectfully returned with the information that the within named man is very useful at this Hospital and under existing orders it would not be proper to discharge him, although he has a g.s.wd of the left hand he is a good [penman?] and performs the duties required of him, as well as a perfectly sound man.
