To his excellency Richard Yates, Governor of Illinois,

    Having been mustered out of the One Hundred Days service, and wishing to continue therin, I respectfully solicit of your Honor, a position in one of the Prisoner Camps, with the same rank which I have recently held (i.e.) Captain.
    My health not being very good, I am not fit for active service for the present.---

I remain, Your most obedient Servant, Capt. Phillip H. Carr late Capt. of Co. A. 141 of Reg. Ills. Vols.

    I Cheerfully reccomend the applicant Capt Cam as a fit person for the position sought for in the above application.
