Before me the undersigned a Notary Public in & for said City personally cause Sarah Williams who being duly sworn deposeth & saith that her son John H. Williams has enlisted in the Service of the United States in Company H. 129th Regt. Illinois Infantry. That he was born 21st day of June 1848 and was only Sixteen years old on the 21st day of June 1864. That he joined said Service without her knowledge or consent, That she is now very old and feeble in health and that said John H. Williams is her only means of support. That she has had three sons in the army who have all been discharged on account of Physical disability.
The said John H. Williams enlisted on the 4th day of August 1864 by the Provost Marshal at Springfield Illinois for one
year in the Company F 129th Regt. Illinois Infantry Volunteers.
That deponent resides near Exeter Scott County Illinois. That the father of the said John H. Williams has died since he enlisted in the Army as aforesaid, and has left her deponent with no one that is able to work, and is very necessary for the said John H. Williams to return to her assistance.

Sarah Williams, being duly sworn, says that her son Jno H. Williams enlisted in the army Co. "F" 129th Ill. That he was only sixteen & enlisted without her knowledge. that her husband had since died & her son is her only means of support. Therefore she wants him discharged.
